What You Need to Know About Support at Home and Assistive Technology Funding

From 1 November 2025, Australia’s aged care landscape is shifting significantly with the launch of the Support at Home program, which replaces the existing Home Care Packages (HCP) and Short-Term Restorative Care (STRC). A key part of this reform is the Assistive Technology and Home Modifications (AT-HM) Scheme, which provides separate, upfront funding for essential products that help older Australians stay safe and independent at home.

What is the Assistive Technology and Home Modifications (AT-HM) Scheme?
The AT-HM Scheme is part of the federal government’s aged care reform under the Support at Home model. It provides dedicated funding, separate from your general care budget, to cover the purchase and installation of approved items and home modifications that support:
- Independence
- Functional mobility
- Safety
- Age-related decline
- Disability management
The AT-HM Scheme officially commenced on 1 November 2025.
Who is Eligible?
You may be eligible for AT-HM funding if you:
- Are
65 or older (or
50+ for Aboriginal or Torres Strait Islander Australians)
- Are
approved for aged care under Support at Home
- Are
transitioning from a Home Care Package (HCP) or
CHSP
Eligibility is assessed via My Aged Care. Most existing HCP clients will be automatically transitioned.
How Much Funding Can You Get?

How the AT-HM Categories Work
Every item in the AT-HM list is grouped by how it should be accessed:

| Under Advice | Safe but may need professional guidance | Should be used under the advice of a provider or clinician. |

Conditional Inclusions
Items that may be funded with clinical or cultural justification
Some assistive technology or home modification items are only available if they are explicitly justified in a participant’s care plan. These are called conditional inclusions — they’re not guaranteed, but they can be funded under the AT-HM Scheme when appropriate.
Domestic life
May be prescribed if evidence is provided to support an assessed need, such as ongoing and severe incontinence unable to be managed.

Assistance dogs
If a participant is not accessing the government-funded Physical Assistance Dogs Program, some of the costs associated with essential assistance dog maintenance may be included under the AT-HM scheme where the services directly relate to the upkeep of the dog.
A dog must meet the definition of an assistance dog used by Health Direct and be required to enable participation in activities of domestic life.
Essential assistance dog maintenance costs may include: animal vaccinations, deworming and flea treatments, essential grooming, dog food, vet bills. It does not include non-essential assistance dog maintenance costs, such as boarding kennel fees, or grooming for aesthetic reasons.
Note: funding for assistance dogs is a specific need with separate funding. Participants may only claim for the following where they have received this separate funding, and the dog meets the definition of assistance dog used by Health Direct.

What About CHSP and HCP Clients?
- Home Care Package (HCP) clients: You can continue to use
unspent funds to purchase approved Assistive technology. Your provider can also
request a Support Plan Review to access AT-HM if needed.
- Commonwealth Home Support Program (CHSP) clients: You are not eligible for AT-HM unless you transition to Support at Home. However, you may be able to access personal alarms via GEAT2GO.
